This page describes the steps to submit a First Notice of Loss (FNOL). Agents will provide essential details about the loss and submit the claim for processing.

Steps to Submit FNOL

  1. Log in to the Equinox system and navigate to the policy requiring a claim submission.
    1. Use the search bar to locate the policy by number, address, or policyholder name.
    2. Confirm that the policy is active and eligible for claims submission.
  2. Select the "Claims" tab or menu option for the policy.
  3. Click the "Submit FNOL" button to start the submission process.
    1. If the button is not visible, ensure the policy meets the eligibility requirements.
  4. Complete the FNOL form with the following details:
    1. **Policyholder Information**: Verify and update contact details as needed.
    2. **Loss Details**: Include the date of loss, cause of loss, and any preliminary damage estimates.
    3. **Property Information**: Confirm the affected property address.
    4. **Supporting Documents**: Upload any required documents, such as photos or third-party reports.
  5. Review the entered information for accuracy.
  6. Submit the FNOL.
    1. A confirmation message will appear with a claim reference number.
    2. Note the reference number for follow-up purposes.

Common Issues

  • FNOL submission errors due to missing policy details or eligibility issues.
    • Double-check that the policy is active and matches the date of loss criteria.
  • Uploading large files may cause delays.
    • Ensure document sizes do not exceed system limits.
